From the doctor who built it

I am an internal medicine physician. I chart all day.

I trained in Miami and Chicago, and I have used every dictation tool that came through those years: Dragon, MModal Fluency, Wispr Flow, Willow Voice, Eloquent, half a dozen others. I watched them get better. Most of them are packed with features. Most of them are also too complicated, or not accurate enough, or not fast enough, or priced like a hospital bought them.

AI changed what is possible, and there is a lot of software now that will save you typing. Almost all of it is either locked to one app, or it works everywhere and charges you every month, forever.

I wanted one thing that was simple, that looked good enough to leave on the screen all day, and that worked in every app I touch. Paid for once. Everything included. Good for anyone who types, and built around the people who type the same paragraphs all day.

Charting is most of my day. I am still at the keyboard long after the last patient has gone home. Anything that gives me an hour back with my family is worth building.

So I built it. One key, no subscription, and my notes never leave my computer.

Measured by the app itself. Every dictation stores its own number in History.

You speak at about 150 words a minute. You type at about 40.

With on-device AI cleanup on, text still lands in under a second.

Say three words. Paste a paragraph.

A block of text you wrote once and named. Say it anywhere in a sentence and the whole block lands in its place, untouched by the AI. A normal exam, return precautions, the sign-off you type forty times a week: two words instead.

Nothing leaves your computer.

Speech to text runs on your computer’s own chip. AI cleanup runs there too. By default Loquen makes no network calls. None.

HIPAABuilt for HIPAA the simple way: patient notes never touch a server, so there is no server to trust.

  • No account.
  • No telemetry.
  • No screenshots, ever.
  • Enhanced mode is opt-in, sends text only, never audio, and your EHR can stay Private by app.

Hold a second key. Identifiers come out.

Hold the Privacy Mode key instead of the dictation key, and Loquen finds and replaces common identifiers before your text is pasted: names, dates and ages, record and account numbers, phone numbers and emails, addresses. It writes them as plain words, and the cleanup pass then rewrites the sentence so it reads naturally. The year of a birth date stays; the rest goes.

  • You see the whole result with every replacement marked. Return pastes it. ⌘Z pastes the original instead.
  • Built around the categories HIPAA’s Safe Harbor method lists.
  • Runs entirely on your computer. An optional higher-accuracy model, about 200 MB, downloads only if you ask for it.
  • For a message to a colleague, a consult by email, a teaching case, a discovery document. It is off unless you hold the key.

A tool, not a compliance programme. Detection is not exhaustive. Review every result before sharing.

Speaks your specialty.

Vocabulary packs spell the words of your field the way you would. Eleven medical packs ship in the box, 4,230 terms, and the AI is told not to “correct” them. Medical core alone is 1,957, built around the drug names you actually write, generic and brand. No more metoprolol coming out as metropolis.

Medicine ships first because that is the job I know. Law, accounting, engineering and the trades are next, and a pack is one plain JSON file, so anyone can write their own.

The questions people ask at this point, answered straight.

Which Macs?
Apple Silicon, M1 or later, on macOS 15 or newer. Intel Macs are not supported. Windows is being built; the site will say when it ships.
Does it need the internet?
No. Speech and cleanup run on your own machine, and it works on a plane. The one network call is a once-a-day licence check that sends your key and nothing else.
Which apps does it work in?
Any app with a text field. It pastes at your cursor, with fallbacks for apps that block paste. If nothing is focused, it copies the text and tells you.
Which key?
Hold the Globe (fn) key by default. Switch to right Option or right Command in Settings. Double-tap for hands-free, Esc to cancel with nothing pasted.
What languages?
English today. Other languages are on the list, not yet in the box.
What happens after the trial?
Dictation pauses until you enter a key. Your settings, phrases, dictionary and history all stay exactly where they are.
How many Macs?
One key works on up to three of your own Macs. Deactivate one in Settings to move it.
What if it is not for me?
Fourteen days, full refund, no questions. Email support and it is done. The trial in front of that means it rarely comes to it.
Does Privacy Mode de-identify my notes?
No, and nothing can promise that. It finds and replaces the common identifiers and shows you every replacement before it pastes; you check the text. It is built around HIPAA’s Safe Harbor categories and runs on your computer, but what you share stays your call.
Is my audio kept?
Only if you turn on the audio archive, and then only on your Mac, so you can retry a dictation. Off by default.
